Good Day List,
I have a 300 lb. new BK patient who was involved in a motorcycle accident.
Patient was first put in a bi-valve post-op custom protective device. This
aided in keeping knee in full extension, helped to allow sutures to heal
safely and allowed us to pro-actively reduce edema. First socket was made
after limb was healed and most edema removed. I performed traditional TSB
socket modifications and everything worked well. The tissue was still pretty
solid over lateral aspect of limb. I took a new impression for a socket
change. Due to his size, 4.5- 5 limb length and the tissue becoming more
flaccid over lateral wall. I have a limited area to load. Made 2 check
sockets and got everything looking very good. Once he started walking on
definitive. He was getting a lot of pressure over lateral aspect of distal
tibia. I am using an Original 6 mm Alpha Locking liner size Large Plus.
Just though before I re-cast him next week I would throw it out there to see
if anyone has had a successful outcome with a similar case?
I am considering positioning the tissue a little different in the liner and
trying to provide equal support over the soft tissue this time. And if
unable to unload properly, may need to move up to an additional thigh
lacer??
