I recently fit a short transradial with a silicone
suction liner (Iceross). The socket design was Otto
bock supracondylar. The reason for using the Iceross
is to create excellent suspension and lengthen the
lever arm of the residual limb with the pin. Your
patient is very short but I think it is worth a try
before going to an above elbow type fitting.

I agree it will be tough fit but you should be able to
fit the 3cm limb. You may end up using a suspension
sleeve if self suspension is not enough. You could
also use a cookie crusher circuit or 1 site 2
channel control. There is a stump activated locking
hinge you could try but this is rather bulky and
cumbersome for a 6 year old.

Aaron,

You may want to try a passive prosthesis to see if she
will tolerate the weight. Then proceed with
myo-electric/electric.
If she is to be fit with a myo electric, I would fit a
silicone liner with pin attachment. The electrodes can
be put through the liner (Motion control technique) or
holes can be made in the liner so Otto Bock electrodes

can touch the skin. Yes, their will be limited range
of motion, but most range of motion with terminal
device placement will come from the glenohumeral joint

(shoulder) anyway. Keep as light as possible.

type of fitting solutions for short b/e residual limbs
vary depending on the activity level of the amputee.
In your case I would suggest a silicone laminated
socket using Ipocon 7 or Orthosil. You probably need
between six to nine layers of perlon stockinette and
possibly some reinforcement in the proximal brim using
felt. You can choose between
a standard child size suction valve from USMC, a
Peewee valve, or a car tire valve. If you want to try
and fit without pulling the residual limb into
the socket with a silky pullsock, you can try a wet
fit by using alcohol or moisturizing lotion. The
socket will need to bonded to the forearm with a high
grade silicone cement.

Hi Aaron,
I don't have a lot of experience with very short
transradial myo fittings, but a locking liner with the
new technique of attaching electrodes may be an avenue
for you to pursue. It would provide a simulated limb
length elongation and a positive, confidence building
suspension. Just a thought.
