Indiana Medicaid and the L5999 codes for Cleg 4

Hello,

Has anyone in Indiana billed Medicaid for the Cleg 4? There are 2 codes
that Otto Bock suggests be billed with the L-5999 code and they provide a
very detailed description for these add-on codes.

I am looking for information or help with Indiana Medicaid (Hoosier
Healthwise) about how likely it will be to get paid for a miscellaneous code
like this? Has anyone billed this way yet, and what was the result?
