Does anyone know where in CMS or OIG or ACA, or any other government agency, it is stated that physicians actually have to see a Medicaid patient and provide documentation to back up an order they write?
For those of you who are experiencing audits with your state’s Medicaid program and being told the physician document was not acceptable, would you please tell me what federal law/regulation they reference in regard to physician documentation requirements not being met?
