If you scroll down on the current LCD you can see a list of revisions
that have been done over the past several years. Even with this, I've
not been able to find the LCD revision for this specific code. We too
are getting recoups for this code. I contacted AOPA and I was informed
the LCD for this code was updated in 2006 to show a K4. I asked for a
link and haven't received it yet, however I am certain AOPA's
information is factual.

patients, which Medicare seemed to pay with out problems. Is there a way

can go back and look at the Medicare LCD policy from 4 years ago?
