Hello All,
We have a prosthetic patient that is going to her physician for prosthetic evaluation. I would like to get it done right the first time. Is there an evaluation form out there that ensures the physician hits all the bases? Any help would be appreciated. Thank you for taking time. Kevin

We have sent the AMP scale and article as well as the PAVET (only if above knee). This helps give the physician what is required for documentation and helps with compliant physician documentation

Read the Dear Physician form and make sure you get all of that information

Kevin, you could make one up similar to the certifying statement for therapeutic shoes. Try and keep it simple but cover all your key points to hit the coverage criteria.

Not only do we have an eval form but ours is an electronic assessment in
which you can assist the physician to complete and then with a click of a
button, it will auto-generate a draft of the physician's notes. The
software is called 'Physician Prosthetic Assessment' (PPA). I am hosting
the next webinar on the PPA software tomorrow at 2pm EST if you are
interested (see attached). I have also attached a price list for the PPA
software.

If you get one, post it. You would think these brilliant people(doctors)
could read a very plainly written list of parameters from the Dear Physician
letter and at least get 1/2 of it documented.

In this game, the bases are always running, so they’re exceptionally hard to hit. As an example, attached is a letter from the Jurisdiction C Medical Director; it is also countersigned by the other three Jurisdiction Medical Directors, which says that it is also policy in their Jurisdictions. The last paragraph is the one that effectively puts the bases in motion. It is very specific and says absolutely NOTHING!
As long as you’re reading, go back and re-read paragraph three at the bottom of page one. It basically that our records are self serving and therefore will not be used to make a reasonable and necessary determination. Yet it can and is held against us if we don’t have them!

Attend the appointment with the patient and assist the doc to fill out what form you end up with. Help the doc learn the importance of this documentation and get him to sign it on the spot.
Great way to connect with the physician and detail him on your services and you get what is needed for billing appropriately. Follow up when prosthesis is delivered and reconnect about
the form and the end result (prosthesis and successful patient). Hopefully you will only have to do this once or twice with each referring physician. As to the proper form I would base it or create it on medicare requirements but am not aware of one yet available.

Send you physician a copy of the Medicare CMS letter to physicians for artificial limbs on Medicares web site. I t outlines all the things that should be in his progress notes. In addition you can sebnd him a copy of your evsluation but do it in the same format as the CMS letter.

Medicare has a listing of what they require, including the attestation of physician's signature form. It should be on CMS website; or you can contact Zita Upchurch at CGS

Thanks to all for the responses. One response has personal information, as they are marketing a document to just what this question asked for. It's no cheap, but what is anymore. There is one more post, sent separately, as his was the only sample provided.
