I believe the most appropriate answers to pass on are that the L2820 code is an addition to a plastic AFO and the L1932 is a carbon fiber, as well as that the L1932 is an off shelf AFO and is not intended to have an additional codes.  I  have used the additional code of L2820 for Kaiser and it is accepted. I guess for Medicare, it does not fit the guidelines.

However, also received responses that is has worked from a practitioner in Maine.  Had a suggestion to use K0672, replacement for soft padding kit.

Had responses from people that have debated trying to bill, but feel it is too inviting of scrutiny.

Original Question - Have had two denials on adding below knee padding on an Allard Toe-Off (L2820) ?  Has anyone had this work?  The padding is purchased separately, so I would think we would get to bill for it also?
