Dear List,
I currently have a 30 year old BKA trauma patient with the worst hair
follicle and sinus tracts on his residual limb that I have seen in my
thirty years practicing in prosthetics. He currently has dozens of
chronic infected hair follicles some draining with interconnecting
sinus tracts. They run as far down as 2/3 the way to the distal residual
limb. I have managed this patient since he was three and about puberty
he began getting the infections. He has been in a sock fit hard socket,
soft inserts, roll on liners. He currently wears a seal-in liner/wave
which he thinks has worked best of any system so far. He has been seen
in wound clinics and by dermatologists and practices good hygiene, he
has used special soaps and washes but the problem always persists. I
recently tried a liner/liner sock under the seal-in which made the
problems worse. He is currently on a round of antibiotics which gives
him some temporary relief. Any thoughts short of electrolysis to help
this patient resolve/improve his problem?
