Hello List

As you know on July 1 2010 new additional required verbiage for DMII
therapeutic shoes . The requirements I am aware of are listed below.
 I am writing to ask where does this fit in the notes [for example a SOAP
format] and how to tersely cover the required verbiage adequately, so we can
get paid.
Also how far afield shall we go as I am not a podiatrist.

1st :
You must document an in-person eval prior to selecting shoes,

Evaluation should include:
1 . Examination of the patient’s feet

2. document any measurements ,casts/ impressions you have made.

3. a description of any conditions that will be treated by the shoes,
inserts, or modifications.

*example :*
protection of existing : peripheral *neuropathy*, edema, amputated toe,
 length discrepancies ,ankle instabilities ,charcot hammertoes, claw toes
bunion

use as prophylactic for ulcers , amputation , *Charcot* foot ,hammer toes,
claw toes , morton's neuroma ,
metatarsalgia.

2nd
You must conduct an in-person delivery subsequent to selecting shoes
In the delivery notes you must document proper shoe/inserts fit and
and that the shoe/inserts meet the patient’s needs.
 ex: shoes are an appropriate fit for DM II presentation and meet
patient's DM II needs , 7mm excess space in toe area,
