Dear List.

We are working with a 50 year old female with a hx of proximal tibia/distal
femoral osteosarcoma. 6 years ago had endoprosthesis placed and then 4 years
ago a revision due to infection. Last year had significant soft tissue
dehiscence and ultimate pedicle flap which is currently failing. She is a
candidate for AKA.

Her surgeon has asked her to consider Van Ness Rotationplasty. Following
lengthy discussion I believe that she is a candidate and we have treated
several rotationsplasty patients BUT, I do not know of this being done on an
adult. So my questions are:

1. Does anyone have any experience with Adults and rotationplasty?
Any concerns about this in adults vs peds population? Anything we need to be
aware of such as increased risk of derotation, slower healing, poor outcomes
etc

2. Does anyone out there have an adult that has rotationplasty
(performed as an adult) , especially if they are female, that my patient may
be able to talk with regarding activities, function, cosmesis, etc. she has
a fabulous attitude but we both feel she would benefit from someone with
experience.

Thank you all so much for your thoughts. Gary
