Thank you to everyone who responded and those who continue to respond. If I receive any further suggestions I will forward them on. The general consensus was to use OPIE software. At this time, we are not looking for another office program. What we have concluded is to make our own template.

Original Post: As everyone knows Medicare is pushing our profession to be more in-depth with medically justifying the Lcodes used on our patients. My question is does anyone have a program or office form with each Lcode, the description and the justification for that Lcode that they would be willing to share.

OPIE has the forms which can be part of the receipt slip or stand alone. The justification can be modified for a particular patient's case. You can create your justifications to meet your company needs.

We use OPIE and in that system, the Lcodes can have default justifications
that can then be modified for the individual patient. You do have to enter
them in the first time, but the system will remember then going forward.

I have learned from attending several AOPA billing and coding courses that
medicare does not like to see generic justifications.
We have recently purchased OPIE software, this has an excellent section for
creating justifications.
They also caution us not to use the same ones over and over, this can create
red flags with Medicare.
So, just a word of caution, remember that the justifications are supposed to
be patient specific.

We use OPIE in this facility, and I really like it, especially for what you are asking about. You can save justifications for LCodes and pick from your bank of justifications when doing a detailed Rx/LMN. You can then tweak each to fit each patient. I'm sure you could contact OPIE (oandp.com) support and perhaps have them print you off a report with the justifications they have on file. Good luck.

Try O&P Surecode. This program does a very good job of creating a list of HCPCS codes used with the HCPCS base code of the item you wish to provide. Somewhat idiot proof.

Your justifications will change depending on the individual needs of each patient and how they present.
Our office runs on OPIE and it allows us to develop and select from a variety of justifications.
It's much cheaper to buy and use the software than it is to return thousands of dollars to Medicare.

Well the not easy answer(it isdefinitelya financial investment!), but the easy solution is of course Opie Patient Management System which helps manage your entire office including yourelectronic medical records. The software generatesprescriptionsusingdescriptions and medical justification which are defined by the practitioner. We purchased Opie software for our practice about 6 years ago and it changed the efficiency of our practice significantly. I highly recommend you have a demo appointment.
