We are looking to provide a form to our patient, to give them information as to what kind of shoes would work best to attach to a DUAFO when purchasing their shoes. We are wondering if anyone had such a form already compiled and wouldn't mind sharing.
The form would list requirments the shoe must have to work with the attachment, ie: no air chambers/pockets, gel, etc.
This would save a lot of returns to the shoe store for our patients.
