Hello,
 
55 year old female with ALS was referred to my office. Extreme neck
extensor weakness. Has tried all OTS cervical collars, and is presently
wearing a L.A. Wire frame orthosis from a different provider. In a
wheelchair much of the time, and a custom designed chair is on the way
which will have a tilt/recline feature so that gravity does not affect
her neck weakness. Out of the chair she is still ambulatory around home
and would like something to replace the present CO. She does not want
any strapping around the chest as breathing is very difficult at the
best of times. I am picturing a device with a plastic posterior TLSO
shell, with an occiput extension and strap that would go around the
forehead. Distal strap to go around waist.
 
Has anyone experience with this type of situation.is definitely my first
go at something like this.
 
Any advice would be appreciated.
