I have some questions regarding obtaining office notes from physicians for
your clinical documentation.

Are you still being required to provide copies of Physicians notes by
insurance companies?

What devices or codes are you still being asked to provide physician notes
for?

Which insurance companies are requesting these from you?

What have your internal policies been to address these issues?

Do you submit claims to insurance companies prior to having physician notes
in hand?

Our internal practices have been on the overly cautious side and we have
been obtaining physician notes for all prosthetic devices as well as for
anything that is being covered by BC/BS or other commercial insurances.
This has been creating a problem at times as we hold up billing procedures
until we finally obtain notes from physicians. I am trying to streamline
the process so that we only obtain notes in the cases where it is absolutely
necessary instead of holding up everything.

I'll post a summary of responses and hopefully come to some conclusions on
what the fastest and most appropriate processes will should be.

Thank you, and I appreciate your feedback.
