Dear List Members,
I have a follow up question on a case I wrote about a while back. I have a patient using a Harmony system who has developed yeast infection repeatedly. The TEC liner was replaced, since the bacteria from the first infection may have impregnated the liner and caused re-infection. Based on input from Otto Bock and this List Server, I decided to replace the liner, rather than attempt to clean it with alcohol, etc. Last week the patient wore the Harmony system socket with an Evolution liner (which is reported to be non-poruous.) I also replaced the suspension sleeve in case it was responsible for re-infection. The patient called today and reported that the brownish drainage associated with past yeast infections has begun to form again in the bottom of the Evolution liner. He is scheduled to see his physician next week.
This is a very compliant and conscientious patient. He has psoriasis. His past prostheses have been TSB sockets with Alpha locking liners. In that system he does not develop the infection problems, but has frequent callousing and skin breakdown. This patient is very pleased with linkage and comfort provided by the vacuum socket. Problems with pressure areas and sores on the residual limb resolve when he wears the Harmony system.
Any ideas about what might be causing the infection, and how to proceed in general?
