The California Orthotics and Prosthetics Association passed a bill more
than ten years ago that prohibits this type of restriction. Although
there is no mandate to cover O&P in California, if a carrier does offer
coverage, it must be provided as per prescription, including replacement
and repair. Of course, insurers do have a right to perform necessary
utilization review, but any contract with the types of limitations you
are addressing, would be illegal in California.
