I have condensed the responses:

Anatomical Concepts, PRAFO, infant 450

Bolt Systems, modify with pad or wedge, articulate as necessary

Boston Soft AFO. They come in 6 ped sizes.

Cascade DAFO has a night Splint that is adjustable

Orthomerica, they custom fabricate them

RCAI, L'Nard Multi-podus Devices

Pediatric Cam Walker, from DeRoyal or Pel, Pel also has PRAFOs, child or pediatric

Wheaton Brace, Lined AFO

OR

Custom Mold over exist or patinet cast, use joints as necessary, add strapps to control dorsiflexion of foot.
