Hanoi 28th May 2001
Dear All,
Thank-you to the many people who have offered us advice on the treatment options for Ms Huong, 7yo with congenital pseudoarthrosis of the tibia. We are still in the process of deciding, with the family what is the best option for her management at this point in time.
If it is at all possible the family would like to retain the foot rather than have an amputation straight away. From the many pieces of advice that we had, there was about a 50/50 split between opting for amputation (at either Boyd, Syme or transtibial levels) or reconstruction using Ilizarov lengthening. There are many factors to add into this equation given Huong’s situation. The surgical options both have many complications and the decision for how to continue will depend first upon the families wishes and then the availability of the treatment within Vietnam, maybe even vice versa!!
There is the possibility of an amputation and good prosthetic fitting here in Hanoi, but the long-term options for this outcome include the social problems of being an amputee. On the other hand, she already has significant scarring on her leg, further surgery will increase that and the outcome of reconstruction may not provide a greater cosmetic solution.
Huong and her family come from a poor farming village 25kms outside Hanoi. Even this close to the city there is little in the way of rehabilitation in the province and it is difficult for Huong and her mother to stay away from their home for too long. For this reason it is hard to start in a long-term surgical and rehabilitation procedure as follow-up will be difficult. The option of amputation also carries similar problems, services are good in Hanoi, but follow-up for prosthetic use and care is also limited in the provincial area.
With so much input in relation to her options we are in a much better position to make an educated choice about her treatment. If you are interested in her progress please contact me directly and I will update you regularly.
