Dear OANDP member,
This is in reference to your problem of genu valgum in hemiplegic
patient
Below are the tips:
Fabricate a brace with long joint on lateral side extending from mid
thigh to heel.To have maximum leverage keep the joint as long as
possible.First try poly centric free knee joint. Due to hemiplegia
having knee contracture if free knee joint does not work out well then
try drop lock knee joint
Fix three well molded polyurethane calf
A) well fixed thigh calf
B) well molded tibial calf ,well contoured on lateral tibial condyle and
patella tandon
C) well molded calf just above the ankle.
Fix the well cushion preferably having air filled insole,sandal with
adjustable velcro strap so as to take care of diabetic foot.
Attach two dynamic force straps(DFS) as under
1) Fix one end at tibial calf(B) ,on lateral side going from posterior
tibial aspect ,pressing with sliding pressure pad on medial tibia
-femoral condyle & buckled on thigh calf) on lateral anterior side .This
strap will tend to correct knee valgus angulation & at the same time
will give medio lateral stability to the knee
2)Another dynamic strap will start from lateral end of calf(C) keeping
pressure pad on mid of tibia and attached on femoral end on calf(A) on
lateral side. This strap will impart more valgus correction at the knee
with medio lateral stability.
For knee contracture try push back knee calf during resting period.
This brace will give you excellent result within couple of days.
