The great thing about this list is that persons with years of experience
offer to assist those of us who are hungry for knowledge. Thanks to all
who responded. Below you will find the responses. Some ideas have been
tried and others will be researched. If I am successful with this
pursuit then I will certainly pass the info along.

Original post:
I have been asked by the father of a 35 year old CP patient to find knee
joints for a KAFO that allow trigger locking and unlocking in any
desired position. Function similar to prosthetic elbows if you get my
drift. He has been to at least two other orthotists and does not
understand that such a joint does not exist. He says no one is listening
to him. Maybe the joint does exist. So do any of you know of such knee
joints please comment. We have tried step lock but he insists on the
alternative locking device.


Many responses were offered that stated: Try Orthotic Technical
Services (OTS) StepLock joint, or Trick Knee by OTS and Becker.

Two respondents stated that they are working on such a joint and expect
to launch it in the near future. They may not be ready for you to beat
down their doors yet so I'll refrain from naming them. I'm sure you will
be hearing from these manufactures in the future.

I never heard of one in the 32years I have been in Orthotics. That is
not to
say one does not exist and if someone knows of such I hope it is shared
with
all of us.

Bill where there is a need there may be a product. Anyway that is what
the patient's father was hoping.

We do not have a joint of that type in the UK but it could be useful
please keep me informed if you find anything

Daw used to make such a joint and may still. I would contact them.

Try looking into the Becker Monodos Joint. I only moves in one
direction, always
locking in the position it moves to, then you pull on the spring loaded
lever to unlock
the joint to go back into flexion. Usually you use them in contracture
management, and
use one on the lateral side and a conventional joint on the medial. I'm
not sure if it
would be strong enough for your proposition, but thought it may help
you.

Jason this joint came to mind initially but it really does not do the
deed.

Sounds like you are describing a joint that would lock at any angle,
preventing both flexion AND extension. What would be the benefit of
limiting
knee extension? I don't think there is an application for such a joint,
and
that's why it probably doesn't exist.

There is a joint being developed by Horton's O&P in Arkansas with the
help of
NASA. It is essentially a stance phase joint that functions as a free
knee
during swing, but stops flexion during stance (extension is possible
during
stance). ... the idea of the SLKJ is to get the best of both worlds.
The design
goals were to design a joint that would: 1) prevent knee flexion during

stance; 2) allow knee flexion during swing; 3) allow knee extension
during
stance; 4) not require manual activation for locking / unlocking; 5)
be
lightweight; and 6) be compact enough to wear under clothing.

The joint you describe DID exist, who knows if it is still available.
You need to get with an old orthotist who was around during the polio
epidemic. I would try Jim Russ in Chicago or the brace shop at Warm
Springs Georgia. If I remember Jim worked at Warm Springs prior to
Northwestern.

USMC used to have what you're looking for but you must specify CP joints
when
you look or inquire.

This is the holy grail of Orthotic knee joints. No one has managed to do
it
yet in a device that is suitable for daily use, and it may not happen
for
some time.
