Hi All,
Below are the responses that i had received . Though not a lot but helped me to decide to start compensating the apparent shortening while casting for spinal brace and shoe Increase .

I would love to hear the responses to this. I have compensated the apparent LLD while still providing the spine brace but only in those patient's the referring MD is ok with providing a lift. Some doctor's do not want me to touch the LLD, but this makes me wonder why not address it? I believe that it also depends on the curve flexibility, a more rigid curve may not be addressed by a lift where a flexible curve may.
Please let me know what you find out!

Hello Riaz ud Din. We use the Rigo-Cheneau style of scoliosis brace and through our cast modifications we try to change the frontal plane tilt sometimes try to translate the pelvis as well to re-align the pelvis so it is closer to centre-line. For example, if you have a right thoracic left lumbar curve and find that the pelvis is tilted so left hip is lower than right, and the head is to left of centre-line, then I would consider adding a left heel raise to reduce tilt of the hips and to encourage the head to move closer to midline. Sometimes it is not as clear as that, but I do draw a centre-line from middle of hips straight up to see where the head is in relation to hips, and use the lift to try to achieve improved balance of head over pelvis in cases where the hips look tilted.

Very good question! I have often wondered that myself. I was taught on
Spinecor to level the pelvis. However I am worried that apparent LLD raise
will make curve worse. I would be very grateful to read the responses you
get on this important question.
