Hello colleagues,
I have a man in his late 40’s, 240lb. lifetime TF amputee, very short due to childhood cancer. The limb is about 7” / 13cm in overall soft tissue length with the distal 1/3 being soft tissue. There is a deep scar running up the back of the leg. The shape of the limb is something like an upside down smurf hat (yes, that’s a clinical term). This client has lots of pudgy soft tissue and for decades he has been successfully fit with a pull-in sock, suction fit with a flexible insert. The fitting has always been complicated but in the end we figured it out.
Lately he has been plagued with greater volume fluctuations and also challenges with moisture, humidity and heat. He is a travel agent and travels lots. When his limb is damp / sweaty it becomes impossible to pull in to the socket. It may be time to move to a roll-on liner (custom) with pin or seal suspension. The pin / lanyard is reliable but challenging because it will typically suspend off the distal 1/3 soft tissue. Seal suspension may prove difficult since he is very conical and short and suction tends to break at sitting. Osseointegration is off the table. Any suggestions or success stories with similar challenging presentation?
