I am not sure if I am replying to this correctly but we use Phoenix Custom Molded. We used to use Levy but they have been taking forever to get things back to us. I am curious about other providers as well if you would be willing to share other answers.
We don't use them for diabetic inserts (not something we provide) But Summit Labs does a nice job on our semi-rigid FOs and they do CNC milled diabetic inserts too.
Anodyne
We use Amfit with their scanning system. I have diabetic inserts in my office in about 2 weeks from when I upload them. I know they just went up on price and I can't remember the new price. It used to be $xx for three pairs of inserts. I am very happy with quality and the amfit system is easy to use and quite impressive to show your patients the images in on the screen. Also interesting to see how the feet can change of the years. We have been using this system for 7 years now so some of my long term patients we can look at seven years of images to compare if something is changing.
We primarily use Dr. Comfort.
I am using Apis/Mt. Emey. Usually a two-week turnaround and around $xx-xx for 3 pair
My name is --------------- Ms CPO. If you don’t find a good alternative hit me up after Christmas we are starting a new venture specifically catering to the O and P market for foot orthoses.
We use Forward Motion Medical for all our arch supports/partial feet and the like. We also have used Hersco as well.
PLS or Surefit
I work with Stridelite in Akron Ohio. We manufacture custom diabetic, accommodative, as well as functional orthotics for O and P facilities as well as for podiatrists. If you are interested in having our company manufacture these items I would be happy to get your company up and running with us.

We use Dr. Comfort. xxxxxxxxxxxxxxxxxx. The quality is good lot ofcustomization options. Timeliness is poor takes 5-10 business days toreceive. Though we are 5 days by ground from where they are located, tothat is about a week lead time then from order to shipping.
Archfitters are ok.
We use Arch fitters out of Oregon or we make our own. Whichever is needed.
