I have a 46 year old male transfemoral patient that had a fem-fem bypass in his twenties. The bypass according to recent imaging is nearly completely occluded. Extensive collateral circulation may be providing adequate circulation at this time. The physiatrist thinks his residual limb pain is due to ischemia. He asked if an elevated vacuum system would improve circulation and decrease his pain. I haven't felt the need for much elevated vacuum except in cases where limb volume fluctuations swing greatly. Any suggestions would be greatly appreciated.
