I have a 3 year old patient that has hypophosphatemic rickets and has the characteristic leg bowing and tibia vara with internal tibial torsion. The orthopod is asking for a, left varus prevention brace with locked knee in extension. The physician estimates the length of need one year. Seems like a long time to lock the knee straight and wonder of the ill effects of this treatment. I'm thinking with a locked KO/KAFO the medially directed force at the knee would be most efficient. After looking at the Atlas of Orthotics it appears the bowing can/has been treated with a single medial upright KAFO, free motion hinge at the knee and ankle and a padded leather knee strap to pull the knee medially. Wondering if anyone can speak to this. Thanks in advance for your responses.
