Hello,

We are students at California State University Dominguez Hills in the
Orthotics and Prosthetics program. The purpose of this message is to invite
you to participate in a research study about the relationship between
perceived access to mental health support resources and depression and body
image.

You may participate if you:

1. Have experienced any upper or lower extremity amputation

2. Are 18+ years of age

3. Are able to read and respond in English

4.Have a computer with access to the internet, and a computer browser
installed

5. Reside within the United States of America

This study will take approximately 15 minute to complete, and will be done
online via Google Forms. As part of this study, you will be asked to
complete the surveys provided.

at any time. This survey may be shared and forwarded to anyone who may fit
the inclusion criteria, no sign in are required, and responses are
completely anonymous.
