Greetings again everyone,
Many responses have asked where this is stated. It is in the AFO//KAFO LCD on the Noridian region A website. This LCD became effective on 7/1/2020. It is on page 26 which is in the policy article section under the heading of miscellaneous. Not sure if this link to the LCD will work but here it is.

I'm in region A and I am wondering how my fellow Practioners are handling the updated AFO/KAFO LCD that states in the Policy Article section:
In addition, if the item is custom fabricated, a complete and clear description of the item, including what makes this item unique, and a breakdown of charges (material and labor used in fabrication). This information should be entered in the narrative field of an electronic claim.
I personally feel this is extremely burdensome. Plus I have serious concerns with Medicare wanting a breakdown of charges.
