We were told today by a representative at CGS that when providing a patient with custom diabetic inserts (A5513) we cannot reuse previous molds and have to remake/recast each year. She referred me to the LCD but could not tell me specifically where it stated this. We have scoured the LCD and the Policy Article and do not see any concrete verbiage that definitively states we cannot reuse molds or have to recast each year.
Does anyone have any insight into this or direct information about this?
