Dear Colleagues,
We have a 62 y/o female pt who presented with a 5 inch shoe lift. She is a
performer and embarrassed to be on stage by the Markell high top shoes and
short leg brace she wears. I believe that she would benefit from a
prosthesis but would like some help in having it approved by her insurance carrier.
Medical justification is required and I would like to know if anyone has
had a similar patient and the notes that were used to obtain authorization.
Any help you could provide would be most appreciated,
