Thank you everyone for your suggestions…
The original question…..
> Does anyone have an idea for an alternative to the Sully Shoulder Stabilizer for use with a much smaller person?

> The size XS is too large for the patient as the upper arm circumference is only 7” - This is for a very small framed adult.
The vast majority of the replies mentioned either the New Options Arm-Adillo or the DMO through Boston Brace.
To fit a DMO, one needs to go through a class. Evidently, there is no one in Florida trained, so referring this case is not an option.
The Arm-Adillo and the Sully have similar sizing, so we will follow another suggestion, a sewing machine, and alter the Sully to fit.
Thank you all for the suggestions
