I have a patient that is post CVA of three years. She has been allowed to develop a 27 degree Plantar Flexion Contracture. She now walks with a steppage gait pattern and has Hyperextension of the knee in late stance. I know that the hyperextension is in correlation with the rigidity of the PF Contracture. Her lifestyle is not one that will accommodate aggressive stretching or correction of this Contracture. In an attempt to get her safer in her gait and increase her walking, what are some techniques that have been used? I was thinking of a 1 wedge at the heel with a rocker sole to help with advancement and thereby avoid hyperextension of the knee. My concern is, this approach creating a leg length discrepancy and a trip hazard. Any thoughts??
