I am posting the responses I received regarding my post about custom foot
orthotic manufacturing companies.
I was surprised at the number of response.
I want to thank all who responded.

central fab in Aguascalientes Mexico and we can make you any foot orthotic,

Hersco labs in New York fabricate a cost effective,well made, and profession
looking foot orthotic.

We still do a LOT of FO's, in house, but when I was using central fab I used
Image Orthopedic, ask for Rick.
We use either ArchFitters or STJ for custom FO's. We do not do diabetic
shoes or FO's for any federal or state payers, no exceptions. Good Luck,

I use Acor Orthopedic in Cleveland Ohio, as they are the best that I have
used in the past and they are priced within the competition.

I have found Archfitters out of Oregon to do a great job and
they are cost effective.

We use comfortfit for cmfo's. Don't know cost.
I have been through several lately. I tried Acor in Cleveland and they are
fine for diabetic inserts but their functional foot orthoses suck. they are
cheapest but you get what you pay for. I liked the fact that they sent me a
new set of biofoams and prepaid shipping label with the 3 sets of inserts
for I believe $90. I found foot logic in uniontown ohio and they do great
with functional orthoses but they aren't cheap. also, they don't give a
break on 3 pair of inserts for diabetics so they end up too expensive to
justify for diabetic inserts. hersco in NY is pretty decent cost and decent
product as well as good customer service. turnaround time is not real fast
but probably worth looking at. I have just made the decision to start doing
mine in-house. not sure if that is good or bad decision but I guess I will
find out.
