Hello everyone:
I'm reaching out to you for some suggestions on how to manage a morbidly
obese transfemoral amputee. I am looking to make the patient a new
prosthesis.
Patient info: 71 y.o. female, 5'0, 300lbs, very loose flesh on residual
limb with a skin fold of flesh at the distal end just above her scar from
the amputation.
Currently the patient utilizes a lanyard system with a liner. The patient's
tummy gets in the way of her donning her liner currently, but she is able
to don it independently. This patient wants to be completely independent
when donning her prosthesis.
The problem with her current system is that the liner folds in where the
flesh fold is towards the scar. Her prosthesis also tends to rotate due to
the mobile flesh. I have added a belt, but of course the patient would
like to not have to wear a belt.
The major factors to consider: loose residual limb tissue, a large tummy
that folds over onto her thighs, volume fluctuation, the flesh fold on her
residual limb, the system must be somewhat simple for the patient to manage
independently, a motivated but not 100% compliant patient.
Please send any suggestions or ideas if you have any experience with a
patient like this. Overall, her goal is to have the prosthesis feel like
it's more a part of her. Her limb volume does fluctuate and she is not
compliant about wearing a shrinker or her prosthesis at all times.
