Okay, here is another case I just received. MD wants a hybrid collar that combines a soft cervical front and a rigid back for an elderly female with an odontoid fracture. He doesn't expect the collar to heal it, just to appease her complaints of collars being too restrictive. My thoughts, after having two cervical fusions myself, is that it will do nothing in the case of a whiplash type injury, such as being rear ended in a car accident. My concerns are this:
1) Is this something that has been done or is available? I have never been asked to do this before. In theory, yes, I could cut a soft C collar and leave it about 2/3 in length and have the posterior wrap around, stitch some velcro to the outside and wrap a firm backing from a standard cervical collar to the posterior section to give him and the patient what they want.
2) Is this able to be coded with standard coding? Has anyone ever tried to do this? Maybe I can cut some thin PE tongue material into the anterior section to give it some rigidity while keeping the front low and off her cheeks and meet possible description requirements. This seems to be more for compliance and her complaints of the collar bothering her but, that's what they are designed to do, remind them not to rotate excessively, provide a static reminder to limit some motion and partially immobilize movement.
That's pretty much the direction I am aiming for unless anyone has some other options or advice on similar cases. Thanks again to everyone for their advice and input.
