Hi everyone,
I was recently asked if there is an recourse providers have when an appeal is dismissed by the QIC. If the QIC (second level of appeal) dismisses your appeal, for any reason, you may request a review of that dismissal by an ALJ. This form can be found here: <URL Redacted>. Use the form titled “Request for Administrative Law Judge (ALJ) to Review Dismissal by the Qualified Independent Contractor.” Form HHS-725. Treat this as a regular appeal and send your supporting documentation, along with a return receipt.
