FYI: We discovered on 12/26 when attempting to use the Medicare automated
system that our PTAN was no longer active. Upon investigation, we were told
that a letter sent to our address by Medicare was returned to them by the
USPS. NSC customer service informed us that we would have to re-apply with
the CMS 855 form and pay the fee to get our PTAN back.
We contacted the Ombudsman at NSC who informed us that we would be
re-instated if we provided a letter from USPS. After a few visits to USPS
and many phone calls, we were able to get a letter from USPS validating our
address. The letter was submitted to NSC and our PTAN was re-activated.
