Hello list,

I have a pediatric patient who suffered a TBI in infancy who is now 12 years old and growing. Over the years we have maintained a corrected position of his feet using AFOs however in the last year we seem less able to apply corrective forces he can tolerate due to his size and continued mid-foot collapse. He presents with a great deal of forefoot adductus and external rotation. He has also begun to develop pinch callusing on the great toe.
My question is: has anyone come up with a good way to provide additional forefoot correction either through strapping or modification technique?
I am using molded poly boots to encompass most of the foot to provide as much correction as he can tolerate. His father is asking if there is a way to control the great toe. We've discussed a toe strap but understand it may be poorly tolerated. Any suggestions are welcomed!
