Hello,

I am reaching out to you because of your expertise in the field of
amputation and Prosthetics.

Our research lab is conducting a research study to gather concepts related
to mobility and functioning that are important to an individual with lower
limb amputation in order to develop an internationally valid mobility
outcome measure based on the International Classification of Functioning,
Disability and Health. This study is currently being conducted across five
continents, and we are the North American Site.

I am kindly requesting that you complete this survey for rehabilitation
professionals. At the bottom of this message is a link to the survey to get
your views on how amputation of the lower extremity affects our patients.
All responses are completely anonymous.
