We have a patient - 3 year old male, TBI and associated tone/spasticity. He
has a very strong extension pattern. Hyperextended knees and severely
plantarflexed ankles.

PT wants to focus on gaining flexion in the knees first, she is unable to
get DF at his ankles and it is not her main concern at this time. With
stretching she can get him to 90 degrees flexion at knees, and her goal is
to gain more over time. So I'm definitely thinking KAFO, solid ankle at
this time, they also want to get him to use these in a stander.

Does anyone have suggestions on what knee joints will work to encourage
flexion, hold him where he is now and slowly increase ROM? I was thinking
step-lock and flip it upside down but I've also heard that it doesn't work
to do that. Experience, suggestions, outcomes would be welcomed!
