Greetings list- our question is in regard to what documentation format is commonly used for orthotic/prosthetic/pedorthic records keeping. We are wondering which format is routinely used and if there is a format that has been beneficial/successful in dealings with 3rd party billing and physicians. We have identified the following formats:





1. SOAP



2. Narrative/paragraph



3. Check boxes (computer template for example)



4. Combination of above



5. Others?
