I am seeking evidence that similar lists have existed and been deleted from Regions A, B & C.  If anyone has evidence of these documentation checklists disseminated by the other 3 regions, or similar lists approved by CMS and utilized by any of the qualified accrediting agencies, could you please share it? It would be most useful evidence in my arguments that the documentation requirements have been unilaterally escalated by CMS and associated reviewers, without any discussion or prior advice to the physicians or suppliers, and should not be retrospectively applied in CMS audit programs. We don't happen to operate in Region D but these lists do reflect the documentation expectation we had been operating under vs. the surprise expectations that are being applied (and applied to the lookback period) in today's reviews.  Thank you Randy!
