Thanks to all who replied - it's great to have so much group experience when you have unique issues.
 
I have posted all the responses below.
 
I have decided to go with the T-handled donning hook with small loops attached to the pull-back strap. I did require a d-ring (pull-back rather than pull over) because of knee weakness she will be loading the proximal strap more than a pull over can tolerate. I think she will be able to use the T-handle for other things as well!

you can try a carbon fiber afo with an anterior strut. This way the patient leaves the afo in the shoe under the shoe insert or Velcro it to the inside of the shoe and the patient will simply slide in and out of afo with no need to pull it on.

 
Try the K Chafe! You get buy them from Cascade Orthopedic Supply.

Cheryl, what creates the need for a d-ring? Weight? PF forces? Could she do a strap to Velcro like pediatrics only longer that allows the strap to go around her leg?

 
 
I have made a loop out of Dacron and attached with a rivet.
 

I've had good luck with strategically placed small Dacron loops and donning hooks. Those familiar with riding boots have seen these. They are like a T-handle Allen wrench bent into a hook. For one gentleman who had minimal use of his hands we combined the hook with a flat loop that went over his forearm/elbow allowing him to pull very hard. It also allowed him to return to feeding his goats. He used the donning hook to carry his grain buckets.
 
You could try a loop for the whole wrist to go through on the strap.
