Thanks to all who responded to my question. The original question is posted
below with a summary of the replies received.
I've looked through all my LCDs and Articles about Lower Limb Prosthetics
and can not find the answer. Can a Nurse Practitioner or Physicians
Assistant complete the required Medicare documentation for a TF Prosthesis
or does it need to be an MD or DO?

Replies (summary)
Most replies stated that Yes, an NP or PA could do the documentation if:
1. They had the legal authority in your state to write a prescription (I am
in Texas which gives them that right). AND
2. They are treating the beneficiary for the condition for which the item is
needed
3. They are practicing independently of a physician
4. They bill Medicare for other covered services using their own provider
number.
There were several references to the Medicare Program Integrity Manual
paragraph 5.5 and one comment which supplied the Texas authority
information.
