Hello, all!

My name is Briana Gascho, and I am a graduate student at Eastern Michigan
University in the Orthotics and Prosthetics department. I am currently
researching the psychological impact of amputation as an independent study
project, and as one aspect of this study, I am interested in receiving
input from *current prosthetists* on several questions regarding care of
amputees through the grief and recovery process. To gain this input, I’ve
created a survey, which I’m asking you to consider filling out.

This survey is completely voluntary and you are in no way obligated to
complete it. By choosing to fill out the survey, you will be giving your
consent, but you may choose to discontinue the survey at any time without
consequence. This survey is anonymous, with no tangible benefit and no
foreseeable risks to you; any identifying information you choose to provide
in your responses will be kept confidential. The survey includes 10
short-answer questions and should take 5-10 minutes to complete; you do not
need to complete every question in order to submit it. The information will
be used to compile a resource for prosthetists and students working with
amputees; at this time, there is no plan to use the information in any
journal publication. Any responses will be greatly appreciated!
