If you have first-hand knowledge of this, and can provide copies of
the documents, you should file a complaint with CMS and OIG.
Additionally, anyone aware of this practice in a state having
licensure should report individual incidents to the license board.
Most board do not have authority to go fishing for wrong-doers.
They can, at best, respond to filed complaints.
