Dear list serve members,
I currently have a challenging case I'm working through and would like input from my colleagues who have run into this situation. Pt is a 50 y/o male s/p TTA in July 2011. Comorbidities include DM II and PVD. Healing time took 5 months and revision is unlikely due to many health concerns.This gentleman has a Hx of falls, two of which resulted in the opening of the incision and subsequent distal anterior adherency. He has had distal tenderness from immediately following the amp that is now exacerbated by ambulating with a prosthesis. The issue is, if the socket is tight enough to provide minimal to no distal WB he c/o distal pain from upward traction on the sides of the limb. If I make the socket larger to alleviated the traction this increases distal WB and therefore pain. I have tried tightening, loosening the socket and a plastazote distal end pad with no effect on pain. Where should I go from here?
