Hello,

My name is Tony Gutierrez and I am a resident prosthetist. Our company
has recently received 2 candidates for below elbow myo-electric
prostheses. I am seeking guidance on the proper documentation necessary
to give our patients the best opportunity for receiving these devices.

What I am currently doing is as follows:
Having an outside OT come in and do a functional abilities and
limitations assessment with no prosthesis.
Assessing all upper extremity ROM and MMT.
Consulting with PT, OT, patient, and family on goals for function of
prosthesis.
Determining patient work and or hobby skill requirements.
Assessing patient myo-site availability and working with OT to develop
muscle group.
Using the current state of science from JPO to guide any additional
assessments.

We have not been denied, but I want to ensure that I have all necessary
documentation to give my patient the best opportunity to receive this
device.
