Good Afternoon List members,
I have a patient who is a short right below knee amputee. He has limited use
of his left hand. He is close to 300 lbs. He has vascular insufficiency in
his limb. He has about a 4 tibia and another 3-4 of redundant tissue
beyond distal end of tibia.
I have used Alpha liners with and without pins. The gel causes him to feel
strangulated just having the liner on. I went to a PTB pelite liner lined
with leather. This removed the strangulated feeling but he has gotten wounds
over fibular head and medial posterior hamstring. I have made the
traditional reliefs and adjustments and he continues to get pressure. Part
of the issue is that he needs the fit to be loose enough to get the pelite
liner on with one hand while not being able to see past his belly. This does
not go on the same each time I am sure. And the socks have wrinkles etc. I
have been suspending with a 6mm ALPS SFR sleeve. This is not optimum,
because he can't or doesn't always get it up on his thigh high enough.
I am sure through the many enlightened out there, I will find the perfect
solution to my woes. :-)
