Is there any peer reviewed literature to support the medical necessity
 for provision of two prostheses, one for daily use and another for
 sport or special activities?

 And.. Has anyone been successful in having a US third party payer
 (other than DOD) provide 2 separate prostheses at the same time?

Answers:*

NO. Apparantlly no one has been successful in having a 3rd party payer
provide two separate prostheses. There were several comments about using
quick disconnect with previous parts or having the patient pay for part of a
prosthesis and duplicating the socket keeping costs down. The policy of the
VA and DOD is to provide multiple prostheses in cases that require them.

Not a single response included or even alluded to the available literature.
