Good Morning list Serve Members,

 

I have a question that seems to be coming up more and more.

 

That is the TISSUE ISSUE.

 

It seems that the use of gel liners, silicone liners, pin and vacuum systems
and total contact has created a socket environment in which the trans-tibial
limb tends to create limb volume loss to happen substantially quicker than
the PTB and sock fit sockets of yesteryear. I have had more limbs lately
that have anywhere from 1.5 to 3 inches of redundant tissue hanging freely
past the distal end of the tibia.

 

 I currently have a young lady with Lupus who after initial bk amputation
had a couple of inches of tissue and took 8 months to have suture site heal,
Decide to have a revision and move the tissue back up over anterior of
tibia. 6 months later the inside sutures would not take and she had to have
a skin graft put on and she still has as much or more tissue distally. She
wants to continue to wear the pin system I fit her with a 9mm Alpha Liner
and she tucks the tissue in the liner and then rolls it on. It works so far.
I know that new graft is going to be Very susceptible to breakdown. I am
just curious how others are working with this type of limb. I have a TISSUE
ISSUE! Help me out!
